Case Study – Atrial Fibrillation

Atrial Fibrillation (AF) and stroke prevention is a high priority for the NHS and indeed healthcare organisations worldwide.


Our work with GP Federations focussed on the Detect, Protect and Perfect aspects of AF:


Detect: Each member practice in the federation was provided with MyDiagnostik medical device to increase the screening efforts to detect AF across its patient population, with a consequent increase in diagnosed AF of over 5%.


Our bespoke methodology utilised clinical pharmacists to undertake clinical audit in patients already diagnosed with AF and undertake Protect and Perfect aspects of care in AF and provide an end to end service for the NHS from audit to patient facing clinics and review.


The service has resulted in many documented cautions and contra-indications challenged and now several practices have achieved 100% anti-coagulation alongside improved systems and process for safe management of continued anti-coagulation prescribing.


An academic evaluation is currently underway and will be published shortly

Case Study – Type 2 Diabetes Mellitus

Diabetes is a metabolically progressive disease and affects approximately 7% of the population in the UK and this is set to rise dramatically with Diabetes UK highlighting that someone is diagnosed with diabetes every two minutes.


This represents a significant burden on healthcare economies and health inequalities continue to exist in localities.


Our work with a GP Federation involved conceptualisation to implementation of one of the largest clinical pharmacist led review of patients living with type 2 diabetes in the UK, with over 1800 patients included within the quality improvement programme.


National and local guidance has failed to keep up with the large cardiovascular outcome trials (CVOTs) undertaken since 2015, despite the significant evidence base and its benefits to patient care.


The aims of the project are to implement the cardio-metabolic pathway and ensure the individualisation of therapies taking into account individual patient phenotypes.


Our initial pilot work in Bradford, West Yorkshire, England has been recognised by the Royal Pharmaceutical Society of Great Britain as a good practice example of pharmacist led care for patients living with diabetes. Click here for further information.
